If you are being treated at Restorative Healing & Massage for the first time, please bring a completed health history form (PDF) to your appointment.

Payment

● Payment is due in full at the time of service.

●Accepted forms of payment are cash, check, venmo and credit cards.

Free

● Cancellations without a minimum notice of 24-hours will incur a fee.

● A $25 fee will be charged for each returned check.

●Fees must be paid in full prior to scheduling your next appointment.
