Health History

If you are being treated at Restorative Healing & Massage for the first time, please bring a completed health history form (PDF) to your appointment.

Payment

Payment is due in full at the time of service.

Accepted forms of payment are cash, check, venmo and credit cards.

Fees

Cancellations without a minimum notice of 24-hours will incur a fee.

A $25 fee will be charged for each returned check.

Fees must be paid in full prior to scheduling your next appointment.
